Phương Tiện Truyền Thông Cùng với việc tôi tham gia Sự Kiện, tôi đồng ý cho phép VCF, CEP America, LLC d/b/a Vituity và từng chi nhánh và công ty con của họ cũng như các giám đốc, viên chức, đối tác, nhân viên, đại lý, người kế thừa và người được chuyển nhượng tương ứng của họ (gọi chung là “Vituity”) sử dụng tên, giọng nói, tuyên bố, hình ảnh và chân dung của tôi. Examen médico Acepto voluntariamente participar en la feria de salud y en los Exámenes médicos ofrecidos por VCF en el Evento. Entiendo que los resultados de los Exámenes que se me ofrezcan son preliminares, solo para mi propia información, no son en modo alguno concluyentes y deben compararse con los resultados de otras pruebas realizadas por mi médico personal para su correcta interpretación. Entiendo que los resultados del Examen no dan lugar a ninguna obligación por parte de VCF y/o MJF, sus empleados, agentes, técnicos o cualquier médico u otro profesional de la salud de proporcionar una interpretación posterior, pruebas diagnósticas, exámenes, tratamiento o cualquier otro servicio médico. Entiendo que los Exámenes no son diagnósticos y que pueden no detectar anomalías que sí detectarían exámenes más detallados. Vituity Cares Foundation (VCF) facilitates free community outreach clinics in communities throughout the country. Healthcare screenings will be offered during community clinics; availability of haircut expertise at community outreach clinics may vary.

I voluntarily agree to take part in the health fair and health Screenings offered by VCF at the Event. I understand the results of the Screenings given to me are preliminary, for my own information only, are in no way conclusive and must be compared with other test results conducted by my own personal physician for proper interpretation. I understand the Screening results do not give rise to any duty on behalf of VCF and/or MJF, its employees, agents, technicians or any physician or other health care to provide further interpretation, diagnostic tests, examinations, treatment or any other medical services. I understand the Screenings are not diagnostic any may fail to detect abnormalities that more definitive screenings would detect. In addition, I understand it is possible that apparent abnormalities would be found to be normal by a more definitive screening. I understand that for conclusive medical diagnosis of any condition, I need to be examined by my personal physician. I understand it is my sole responsibility to: Provide the Screening results to my personal physician; Follow up with my personal physician on any potential abnormalities detected or not detected by the Screenings and to obtain a medical examination by my personal physician related to the Screening findings; and Carry out any other recommendations or advice regarding the Screening results. On behalf of myself and my heirs, successors, assigns and personal representatives, I hereby discharge, release and agree to indemnify and hold harmless VCF and MJF and their employees, agents, affiliates, officers, directors and representatives from any and all liability whatsoever for any and all damages, losses or injuries (including death) which may arise from my participation in the Screening and/or from the information provided to me concerning the Screening and/or from any other information provided to me by VCF and/or MJF in connection with the Screening.

In association with my participation in the Event, I consent to the use of my name, voice, statements, photographs and likeness of me by VCF, CEP America, LLC d/b/a Vituity and each of their affiliates and subsidiaries and their respective directors, officers, partners, employees, agents, successors and assigns (collectively “Vituity”). Included in this Consent are motion picture, videotape and related representations involving use of my voice and/or pictures of me. Vituity shall be the absolute owner of any and all photographs, recordings and other materials (and all rights therein, including copyright) to which this Consent applies. Vituity may use my name, voice, statements, photographs and likeness of me for any and all purposes, including but not limited to use in public benefit activities, promotional materials, health education materials, public relations, professional education, and art, and in all media, including electronic/digital and print media, without restriction as to frequency or duration of usage and without compensation to me.

I voluntarily agree to have my hair cut by VCF at the Event. On behalf of myself and my heirs, successors, assigns and personal representatives, I hereby discharge, release and agree to indemnify and hold harmless VCF and MJF and their employees, agents, affiliates, officers, directors and representatives from any and all liability whatsoever for any and all damages, losses or injuries which may arise from having my hair cut at the Event.
